2024第十四屆台灣醫學黴菌培訓網研討會
成果報告
台灣醫學黴菌培訓網研討會自2023年起恢復實體會議，本年度議程適逢修訂新的黴菌用藥指引，將研討會主題區分為4個部分：黴菌病的診斷及治療指引、跨學科診斷管理、以及從研究到臨床應用：「Section 1: Antifungal Guidance update」、「Section 2: Guidance update (cont.)」、「Section 3: Multidisciplinary」以及「Section 4: From bench to bench side」。本次邀集台灣醫學黴菌學的醫師及專家，講述不同黴菌病最新修訂的指引，並在section 3 中特別三度邀請香港中文大學微生物系教授 Prof. Patrick Woo 深度介紹醫學上重要的真菌分子鑑定新方法。
會議在感染症醫學會張峰義理事長致詞後揭開序幕。為了深入解說「Section 1與 Section 2: Antifungal Guidance update」黴菌病的用藥指引，今年的課程規劃略做調整，將每節演講時間延長為50分鐘。首先由台大陳抱宇醫師介紹「Candida」、高長陳怡君主任介紹「Cryptococcus」與高醫林尚儀主任介紹「Trichosporon」，由北榮黃祥芬醫師介紹「invasive aspergillosis」，成大吳綺容醫師介紹「Mucorales」，分別針對上臨床的黴菌疾病，從個案介紹出發，帶出藥物治療的原則，並說明指引修正的重點。由於invasive aspergillosis 好發於頭頸部，因此特別在這個主題中邀請北榮耳鼻喉頭頸醫學部主治醫師葉建甫，分享此類病人病灶與疾病特性。
「Section 3: Multidisciplinary diagnostic stewardship」跨科學診斷管理是近幾年盛行的臨床診斷策略，在抗生素的管理中，實驗室專業的醫檢人員的角色尤其重要。由於抗生素抗藥性議題已延燒多年，至今仍然無解，抗黴菌藥亦不例外。黴菌感染的診斷通常較細菌更為困難，在這個主題中的第一個講題，由林口長庚孫培倫醫師介紹在治療皮膚真菌感染時，整合診斷的重要性。第二個講題邀請中山醫大的耳鼻喉科盧彥廷醫師介紹「Invasive fungal rhinosinusitis: Diagnosis and treatment」，最後由輔大重症醫學部劉偉倫主任，從胸腔科醫師的角度，針對嗜中性白血球低下與非嗜中性白血球低下的病人的「Effective diagnostics for both neutropenic and non-neutropenic patient groups – Chest Perspective」，並討論如何在目前胸腔科醫師吃緊的狀況下，進行有效的診斷。
第四個主題「Section 4: From bench to bedside」：從實驗室到臨床端的應用。第一個講題「Application of MALDI-ToF MS in the identification of dermatophytes」，由孫培倫醫師從實際案例中，說明皮癬菌正確鑑定的重要性，並介紹MALDI-ToF在鑑定上的臨床應用。接著由Prof. Patrick Woo 介紹「Molecular identification of medically important fungi」，說明分子鑑定的重要性及困難點，並介紹次世代定序在診斷真菌感染上的潛力。陳抱宇醫師在他的臨床經驗與研究基礎下，分享臨床上重要的酵母菌體外藥敏測試「In vitro susceptibility testing of medically important yeasts」介紹。在治療侵襲性黴菌病的用藥，我們邀請到台大癌醫中心林淑文主任，從長期觀察與研究中，說明從黴菌病藥的藥物動力學，講題為「Updates and challenges of application of PKPD in the treatment of invasive fungal infections」。最後壓軸，由童綜合醫院林國璽醫師講述「One health perspective of Cryptococcus」的講題。全球暖化造成極端氣候，直接影響了農業、植物、動物的生態。林醫師以Cryptococcus 為例，說明環境變化與人類疾病和抗藥性的關聯性。One health是當前世界衛生組織 (World Health Organization) 的重要議題，人類、家畜、和野生動物健康之間聯繫，說明了疾病不只威脅人類，還會對生物多樣性與生態系統造成破壞。保護地球免於疾病的威脅，是我們大家都需要重視與面對的課題。
本屆報名人數總計231人，出席率:77.9%。身份別以醫師159人(68.8%)最多，其次是醫檢師60人(26%)，藥師7人(3%)，其他職業5人(2.2%)。
問卷回收139份，回收率77.2%。經統計後，在會場安排、講題與時間安排上，【非常滿意】的比例較去年提升。已參加過2-5次會議的人數增加8%，97.8% 認為本次會議主題有幫助。另，學員回饋希望加入「形態學與鑑定」，已列入2025年的議程參考。
明年將辦理第十五屆台灣醫學黴菌培訓網研討會，我們繼續致力把最新的黴菌醫學知識帶給大家，持續優化網站內容，將黴菌課程或有關連性的研討會資訊提供給學員。最後，感謝輝瑞大藥廠多年來支持本研討會的辦理，期待明年再相會!
